By opting in to receive SMS messages from us, you agree to these Terms and Conditions (Terms).

 

SMS Messaging Service

By providing my phone number, I consent to receive SMS text messages from [Legal Company Name]  for appointment reminders, marketing messages, and general two-way communication about [insert the product or service provided]. Message and Data Rates may apply.

 

Message Frequency

You will get more than one message from us unless you opt-out, and while messaging frequency varies, you will likely receive up to 4 messages per month. Epiphany Integrative Mental Health  reserves the right to alter the frequency of messages at any time to increase or decrease the total number of messages.  Epiphany Integrative Mental Health and carriers are not liable for delays or undelivered messages.

Cancellation/Opt-Out Instructions

  1. HIPAA Disclaimer: Text messaging is not a fully secure method of communication. While we take precautions to protect your privacy, there is a risk that health information may be intercepted or accessed by unauthorized parties. By choosing to communicate via text, you accept this risk.

  2. Message Guidelines:

    • Text messages should not be used for urgent or emergency matters.

    • Response times may vary; for immediate concerns, please call our office at 720-504-8129.

    • Do not share sensitive personal or medical information via text.

  3. Emergency Situations: If you are experiencing a medical or mental health emergency, do not use text messaging. Instead, call 988, 911, or go to the nearest emergency room.

  4. Opting Out: You may opt out of receiving text messages at any time by replying STOP to any message. Standard messaging and data rates may apply.
